Jayhealth Family Medicine Clinic provides comprehensive primary care services designed to support patients at every stage of life. Our services include routine wellness exams, preventive screenings, diagnosis and treatment of acute illnesses, and ongoing management of chronic conditions such as diabetes and hypertension. We also offer vaccinations, basic laboratory testing, medication management, referrals to specialists when needed, and coordinated care for hospital stays and follow-up visits. Our goal is to deliver accessible, patient-centered care in a comfortable and supportive environment.

What to Bring

To help ensure a smooth and efficient visit, please bring a valid photo ID, your health insurance card, and any referral or authorization documents if required by your insurance plan. Patients should also bring a list of current medications, including dosages, and any relevant medical records or test results. If you are a new patient, arriving a few minutes early to complete paperwork is recommended.

Valuables

For your safety and peace of mind, patients are encouraged to leave valuables such as jewelry, large sums of cash, and other personal items at home. Jayhealth Family Medicine Clinic is not responsible for lost or misplaced personal belongings brought into the clinic. Please keep essential items with you at all times during your visit.

Be involved in your health care

Taking an active role in your health care leads to better outcomes and a more positive experience. Patients are encouraged to ask questions, share concerns, and fully understand their treatment plans, medications, and follow-up care. By working closely with your healthcare provider, you can make informed decisions, manage your health more effectively, and support long-term wellness.

    • 3. Bring all of your medicines with you when you come to the clinic for your medical appointment.
    • 4. Tell a member of your health care team if you have ever had an allergic or bad reaction to any medicine or food.
    • 5. Make sure you know what to do when you leave Jayhealth Family Medicine Clinic after your appointment.

Patient meals

As a family medicine clinic, Jayhealth Family Medicine Clinic does not provide on-site patient meals. Patients with dietary needs related to medical conditions are encouraged to discuss nutrition and meal planning with their healthcare provider, who can offer guidance and recommendations to support overall health and recovery.

    • Fasting Appointments: For certain blood tests, please do not eat or drink (except water) for 8–12 hours prior to your visit.
    • Diabetic Patients: Please consult with our staff when scheduling to ensure your appointment time aligns with your meal and medication needs.
    • Local Amenities: Our clinic is located near several dining options in Arlington if you need a meal immediately following your appointment.
